My take: Interesting to learn a bit about the volunteer work that is done in "doctors without borders style" in other countries. Adrian's ex-wife uses their son Sean to extort money from him while Adrian is away working to heal under-privileged children. Caprice, unaware that Adrian's son has been taken, is upset at the doctor's apparent lack of professionalism at wanting to skip out on his pledge to volunteer.
